resultater. Hjemmebasert støtte og veiledning for tryggere bolig og

med NAV, bolig og helse (multi-agency) - felles målplan og oppfølging
i hjemmet. Opplæring og kurs for kommunale «orden-trenere», pårørende
og fagpersoner (praktisk, ikke-klinisk). Måling av sosiale resultater
og anonymisert informasjons-/destigmatiseringsarbeid (årlig
åpenhetsrapport).
